REGISTER?

We are pleased to register new patients moving into our practice area. Please bring your completed medical card to reception (or ask for Form GMS1). You will be asked to fill in a new patient questionnaire which will give us information about you before your medical records arrive.

If you would prefer to be registered with a particular doctor, please let the receptionist know when you register.
