* You can also obtain an up-to-date BP measurement here at the surgery by using the apparatus in the waiting area. The receptionists will be pleased to assist you if necessary.

CHARGEABLE ITEMS

There are fees chargeable at British Medical Association standard rates for certain services - eg private health claim forms, passport certificates, holiday cancellations, driving and private medical examinations, sickness certificates in first seven days of illness.

2 Months Combined Diphtheria/Whooping Cough/Tetanus (DPT+Hib) Polio (by mouth) and Meningitis "C"
3 Months Combined Diphtheria/Whooping Cough/Tetanus (DPT+Hib) Polio (by mouth) and Meningitis "C"
4 Months Combined Diphtheria/Whooping Cough/Tetanus (DPT+Hib) Polio (by mouth) and Meningitis "C"
12 - 18 Months Measles/Mumps/Rubella (MMR)

If you have any doubts about any of the immunisations please speak to either your doctor or one of our health visitors.

Referral Information For Patients

When we refer patients to other services the referral is administered by the local NHS in a referral management centre (RMC).  The referral management centre is responsible for allocating your referral to other services and normally sees full information about the referral, including any confidential information.

If your GP is referring you to other services and you feel there is any information you wish to withhold from the referral management centre, please inform your GP (or the practice secretary) as soon as possible during normal working hours.

If you experience problems with the referral management centre or wish to discuss its role further please contact the Patient Advice and Liaison Service (PALS).
